This information may include your name, email address, date of birth, and phone number. The information is required to confirm program or benefit eligibility and prevent insurance and benefit fraud. Your Sponsor Organization or the subscriber of your benefits will not be provided with this information if you have already given them similar information in your employee file or record, unless you are a family member of an eligible employee.
